Access and submit our patient forms

Accessing our forms

For your convenience, we’ll email you any intake and consent forms after your appointment is scheduled. This email will contain a link and a security code that directs you to the right forms and ensures that only you have access. Simply:

  • Click on the link
  • This will open the secure access page in your website browser
  • Enter the security code from your email
  • You’ll then be directed to your forms

At that point, you just need to decide how you’d like to complete and submit them.

Completing & submitting our forms

If you’d like to complete your forms online (saving paper, ink, and time), you can do so using any device. Our online forms are HIPAA compliant, mobile-friendly, and even support e-signatures. Just submit them when you’re done, and we’ll be immediately notified.

If you prefer downloading the forms instead, simply complete them ahead of time and bring them with you on the day of your appointment.

Lastly, if you choose to fill out the patient forms at our office, plan to arrive 10 – 15 minutes before your appointment to complete them and take advantage of your full treatment time.
