How do I schedule an appointment?
To request an appointment, visit our scheduling page and complete the form you see there. You’ll share a little information with us, as well as your preferred date and time. We’ll contact you after that to set up an official appointment date.
Where do I access and upload my patient forms?
Visit our forms and uploads page for a full explanation of how we handle this. As a brief preview, you’ll receive access to your forms by email after you schedule an appointment. If you’re a patient in our Artistic Athletics program, we prefer that you fill them out completely online. If you’re a patient at our Moon Township facility, you can choose to complete your forms online, offline, or in our facility.
